Instructions for Submitting a Fellow Award Nomination

Thank you for considering a nomination for the CHM Fellow Awards. To ensure a complete and accurate submission, please carefully follow the steps listed below:

  1. Review the selection criteria to ensure the nominee(s) meet the requirements
  2. Include supporting and comprehensive documentation that supports the  nomination.
  3. Submit the nomination by completing the form below. All required fields must be completed for the nomination to be considered. Please ensure the nominee’s achievements are well-documented and that all supplemental materials are properly labeled and uploaded in the appropriate format.

If you are nominating a group of co-contributors, please submit a separate nomination form for each nominee.
